How do you lose weight on your plan while still taking medication for chronic illnesses eg. Epilepsy?

I have Epilepsy and Rheumatoid Arthritis (52 years old) and have started on a more healthy plan of eating over 1 1/2 years ago. By changing my eating lifestyle i have lost 38kg (was 122kg height 1.65m). I have now come across your website and have read your e-book and found exactly what I am looking for. But I am very interested to find out how much my medication is interfering with my desire to be as healthy as possible and to lose as much unnessary weight as possible. I was taking Topamax 200mg bd but have been able to reduce down to 125mg thanks to my weight loss (now I am 84kg) I am also on Rivitrol .5mg bd. and Trepeline 75mg nocte. For the Rhemumatoic Arthritis I have managed to go off the heavy medication and I now manage on Coxflam 75mg once a day.The arthritis is worst in my in my knees and joints but when I maintain a healthy raw diet this pain is minimal. According to the tests in your book (Metabolism Makeover)my adrenals are somewhat badly shot! I have started on the detox with broths and just raw vegetables but am finding it really difficult as winter is starting - but will persevere as I want to be as healthy as I can for my body!! I am so tired of being ill and feeling that my body is broken. The change of diet has helped alot but I now need to commit to a higher level and higher goal. Please could you help?
